Required records
- Clinic name and dentist name
- Patient reference
- Product type (e.g. monolithic zirconia, e.max)
- Tooth number
- Shade
- Full arch scan or impression
- Opposing arch scan or impression
- Bite record
Recommended supporting info
- Intraoral photos for aesthetic cases
- Shade photos (buccal, incisal)
- Preparation shade if relevant
- Special instructions or aesthetic notes
Commonly missing
- Bite record
- Opposing arch
- Shade or shade photo for anterior cases
- Tooth number (not just "upper right")
Production start rule
Production begins once all required records are received and reviewed. Cases missing required information will be held until complete.
